service provider who has to submit an application to determine the fee shall submit an application to the Commission in the prescribed format along with the following details regarding the determination of the fee:-
(a) Basis on which the proposed tariff should be fixed,
(b) If there is a change in the rate of duty, the basis thereof,
(c) Comparative price list prepared in such a way that the tariff charged in the past and the proposed tariff rate are clearly visible,
(d) reasonable profit that can be made to the service provider from the fee proposed by the service provider, including the service operation, maintenance and storage expenses, depreciation deductions, payment of interest on the loan taken by the service provider for the service operation, and the expenses that increase from the annual price increase,
(e) If the proposed fee is not sufficient to meet the expenses and earn profit as per clause (d) or if the fee should not be increased immediately, the financial resources and long-term plan identified by the service provider to cover such insufficiency,
(f) Comparative analysis of annual increases in service operating expenses and proposed tariffs,
(g) The basis prepared in relation to exemption of tariffs for consumers who cannot pay the proposed tariffs and the additional burden that may be imposed on other consumers due to such exemption tariffs,
(h) If the service provider has obtained a license to operate the service from any body established in accordance with the prevailing law or has entered into an agreement, a copy of such license or agreement,
(i) Other matters as specified.
